Azucena's Primary Home Care
Personal Assistance Services (PAS) are the core of in-home care, covering the everyday tasks that maintain dignity and independence at home. At Azucena's Primary Home Care, our PAS caregivers are trained and supervised under Texas HHSC 26 TAC §558 and verified through DPS, EMR, and NAR background checks.
Adults and children enrolled in STAR+PLUS HCBS, STAR+PLUS State Plan, or PHC (Primary Home Care) Medicaid programs. Also available through Driscoll Health Plan and through private pay for families not on Medicaid.
