Azucena's Primary Home Care is a Home and Community Support Services Agency (HCSSA #024195) located in Brownsville, Texas. We are a covered entity under the Health Insurance Portability and Accountability Act (HIPAA, 45 CFR Parts 160 and 164).

We collect Protected Health Information (PHI) including: personal identifying information, medical history, care plan, visit records (EVV), and insurance billing data.

3. How We Use and Disclose Your Information

We may use or disclose your PHI without authorization for:

• Treatment: coordinating your care among providers
• Payment: billing Medicaid/STAR+PLUS and other payers
• Operations: quality oversight, training, administration
• Legal requirements: mandatory reporting to HHSC and other government agencies
• Public safety: prevention of abuse, neglect, or imminent danger

You have the right to:

• Inspect and obtain a copy of your PHI
• Request amendments to your PHI
• Request restrictions on use of your information
• Receive confidential communications
• Obtain an accounting of disclosures we have made
• Receive a paper copy of this notice at any time

We are required by law to maintain the privacy of your health information, to provide you this notice, and to notify you following a breach of your unsecured PHI.
