As a client of Azucena's Primary Home Care, you have the following rights guaranteed by Texas HHSC regulations (TAC §558.803):
01Receive services that are respectful and considerate of your dignity and individuality
02Be informed, with reasonable advance notice, of changes in services that may affect your well-being
03Participate in the planning of your care and in changes to the service plan
04Privacy in the delivery of services
05Confidentiality of your health information (HIPAA)
06Be informed about available services, their costs, and whether they are covered by Medicare, Medicaid, or other sources
07Receive information about the agency, its personnel, and its policies
08Refuse treatment and be informed of the consequences
09Freedom from discrimination on the basis of race, color, national origin, disability, sex, or age
10File complaints about care or treatment without fear of retaliation
11Be notified of any change in your care plan with reasonable advance notice
12Have your property respected and protected
13Be free from any form of abuse, neglect, or exploitation
14Receive clear information about your rights and responsibilities at the start of service
